Fill out the required form (youth or adult) and send in the additional required information (outlined in application) to ROC Wheels, P.O. Box 11765, Bozeman, MT 59719. There is limited space so get your application in early.
Youth Application
Application for those 18 and under.

Application for those 19 and older.

ROC Wheels gives the gift of mobility. On a trip to Mexico we had the honor of serving Jesús and giving him a wheelchair.
